摘要

目的观察应用ABCLOVE嗓音康复训练法治疗甲状腺癌术后单侧声带麻痹所致声音嘶哑的临床效果。方法选取甲状腺癌术后持续声音嘶哑超过12个月、电子喉镜检查证实为单侧声带麻痹患者36例，其中男性14例，女性22例，年龄18～65岁，平均年龄43.3±8.47岁，病程13～36月，平均17.57±4.90月。所有患者均采用ABCLOVE嗓音康复训练法进行康复训练，每周2次，共训练24次。治疗前后采用GRBAS评估、嗓音障碍指数量表、电子喉镜检查、空气动力学检查、计算机嗓音分析等进行主客观评价治疗效果。结果应用ABCLOVE嗓音康复训练法进行嗓音康复治疗后，36例患者嗓音功能较训练前均有所改善，总嘶哑度、粗糙度、气息度和紧张度评分均较训练前显著降低（P<0.05）。嗓音障碍指数量表各维度评分和总分及基频微扰、振幅微扰均显著低于训练前（P<0.05）。空气动力学检查最大发音时间和最长发声时间均较训练前显著延长（P<0.05）。电子喉镜检查示嗓音康复训练后患者患侧声带活动性较训练前无明显变化，但声门闭合程度较训练前改善。结论针对病程超过12个月甲状腺癌术后单侧声带麻痹造成的发音障碍，可通过嗓音康复训练法进行有效地个体化治疗，从而改善声音嘶哑等发音障碍问题，提高患者生活质量。

Abstract

Objective To observe the clinical effect of ABCLOVE voice rehabilitation therapy on the hoarseness caused by unilateral vocal cord paralysis after surgery of thyroid cancer. Methods 36 patients with continuous hoarseness over 12 months after thyroid cancer surgery and unilateral vocal cord paralysis confirmed by electronic laryngoscope were selected. There were 14 males and 22 females patients

aged from 18 to 65 years old with an average age of 43.3±8.47 years old and the disease course was from 13 to 36 months

with an average course of 17.57±4.90 months. All of the patients underwent the speech therapy with ABCLOVE voice rehabilitation training method twice a week for an average of 24 times to improve the voice quality. Before and after the treatment

examination of Aerodynamics as well as computer voice analyzer were used to evaluate the subjective and objective therapeutic effect. Results Before and after ABCLOVE voice rehabilitation therapy

the voice quality of 36 patients was improved compared with that before the training and the total hoarseness

roughness

breath degree and tension degree were all decreased compared with that before the voice rehabilitation therapy（P<0.05）. The score and total score of each dimension of the voice disorder index scale

as well as the fundamental frequency perturbation and amplitude perturbation of the computer voice analysis were lower than those before the voice training（P<0.05）. The maximum articulation time and maximum articulation time of aerodynamics examination were both longer than that before training（P<0.05）. Electronic laryngoscope showed that there were no significant changes in vocal cord activity after voice rehabilitation training

but the glottis closure was improved compared with that before training. Conclusion The dysphonia caused by unilateral vocal cord paralysis after thyroidectomy with a course of more than 12 months

could be improved through voice rehabilitation training

so as to improve the quality of life of these patients.
