目的

探讨耳鸣感知和感受与主观听配能的相关性。
方法

对72例以耳鸣为第一主诉的成年初诊患者进行纯音听阈、耳鸣匹配检查、残余抑制检查、耳鸣功能指数量表(tinnitus functional index,TFI)测评和2个听配能问题询问。根据超高频(8000、10000、12500、16000 Hz)平均听力和残余抑制效果将受试者分组,对数据进行统计分析。
结果

健听组和听障组间耳鸣感知(即响度感觉级和频率)无明显差异(

0.05),听配能问题1和2评分在组间未见显著差异(

0.05)。健听组和听障组间耳鸣功能指数量表听力分量表得分呈现统计学差异(

0.05)。两个听配能问题在健听组和听障组中分别与不同的耳鸣功能指数分量表得分相关,但均与总分显著相关(

0.05)。残余抑制效果不同的受试者之间耳鸣功能指数总分无显著差异(

0.05),但残余抑制效果对听配能问题1的评分有影响,呈显著差异(

结论

对耳鸣强度和性质的感知可能并不影响患者转移注意力的能力和意愿。耳鸣给患者带来的听力和情绪方面的感受成为驱动注意力转移的重要因素。
Objective

Exploring the relationship of the tinnitus perception and experience with subjective listening effort.
Methods

Seventy-two newly diagnosed adult patients with tinnitus as their primary complaint were recruited. Extended pure tone test
tinnitus matching test
residual inhibition test
Tinnitus Function Index (TFI) and two listening effort questions were administered. Participants were divided into groups based on the averaged threshold across the extended high frequencies and the residual inhibition outcomes. Pearson correlations and ANOVA analyses were performed on the data.
Results

No significant difference was found in tinnitus perception (i.e.
loudness level and frequency) between the normal hearing group and the hearing-impaired group (

0.05)
nor in the scores of listening effort question 1 and 2 between the groups (
P
>
0.05). There was a statistically significant difference (
P
<
0.05) in the hearing subscores of TFI between the two groups. The listening effort question scores were significantly correlated with different TFI subscales between the normal hearing and hearing-impaired groups. There was no significant difference in the total TFI score among subjects with different residual inhibition effects (
P
>
0.05)
but the residual inhibition outcome had a significantly impact on the score of the listening effort question 1 (
P
<
0.05).
Conclusion

The perception of the tinnitus loudness and frequency may not affect patients' ability or motivation to allocate less listening effort on the tinnitus. In contrast
the experience of the impact of tinnitus on daily function and emotions may play an important role in effective listening effort management.
