目的

比较双侧人工耳蜗植入和双模式干预对低龄语前聋患儿的临床疗效。
方法

回顾性分析本院耳鼻咽喉头颈外科2019年7月~2021年6月收治的88例低龄语前聋患儿临床资料,根据干预方法分为A组(n=45)和B组(n=43),A组接受双侧人工耳蜗植入干预,B组接受一侧植入人工耳蜗、对侧配戴助听器干预。分别于开机前、开机6个月、12个月时采用婴幼儿有意义听觉整合量表(infant-toddler meaningful auditory integration scale,IT-MAIS)、简易版普通话早期言语感知测试(low-verbal mandarin early speech perception test,LV-MESP)评价患儿听觉能力和言语感知能力;采用言语空间听觉质量量表-父母版问卷(speech, spatial and other qualities of hearing scale for parents,SSQ-P)评价患儿空间听觉能力,采用中文版人工耳蜗植入儿童家长观点调查问卷(mandarin children with cochlear implants:parental perspectives,MPP)评价患儿生活质量。
结果

B组患儿开机6个月时的IT-MAIS和LV-MESP得分显著高于A组(

0.05),但两组开机12个月时的IT-MAIS和LV-MESP得分无显著差异(

>0.05)。B组患儿开机6个月时SSQ-P量表中空间听力和听力质量维度评分显著高于A组(

0.05),两组患儿不同时间言语感知维度评分和开机12个月时的空间听力和听力质量维度评分比较均无显著差异(

>0.05)。两组患儿开机12个月时的MPP量表各维度评分均无显著差异(

结论

双模式干预治疗低龄语前聋患儿的短期疗效优于双侧人工耳蜗植入,但二者长期干预效果接近,临床应根据手术风险、患儿是否为对称性耳聋、患儿家庭经济情况进行合理选择。
Objective

To compare the clinical efficacy of bilateral cochlear implantation and bimodal intervention in the treatment of young patients with prelingual hearing loss.
Methods

The clinical data of 88 young children with presyllabic deafness admitted to our otorhinolaryngology head and neck surgery department from July 2019 to June 2021 were analyzed retrospectively. According to the different intervention methods
they were divided into Group A (n=45) and group B (n=43). Children in Group A received bilateral cochlear implant intervention
while children in Group B received unilateral cochlear implant intervention and contralateral hearing aid intervention. The children's hearing and speech perception ability were evaluated with the Integrated Meaningful Auditory Scale for Infants (IT-MAIS) and the Simplified Mandarin Early Speech Perception Test (LV-MESP) before
6 months and 12 months after switch-on. The children's hearing and speech perception abilities were evaluated with the Speech Space Hearing Quality Scale Parent Parent Version Questionnaire (SSQ-P)
and the children's quality of life was evaluated with the Chinese version of CI Children's Parent Opinion Questionnaire (MPP).
Results

The percentage of IT-MAIS score and LV-MESP score in Group B were significantly higher than those in Group A at 6 months of boot (

but there was no significant difference between the two groups at 12 months of boot (

0.05). The spatial hearing and hearing quality dimension scores of children in Group B were significantly higher than those in Group B after 6 months of boot (

0.05). There was no significant difference between the two groups in speech perception dimension scores at different times and spatial hearing and hearing quality dimension scores at 12 months of boot (

0.05). There was no significant difference between Group A and Group B in all dimensions of the MPP scale at 12 months (

Conclusion

The short-term effect of bimodal intervention is slightly better than that of bilateral cochlear implantation in the treatment of young children with prelingual deafness
but the long-term effect of the two interventions is similar.Clinical selection should be made according to the surgical risk
whether the child is symmetrical deafness
and the family economic situation.
