摘要

目的

分析语言发育落后儿童常见发育性疾病的智能发育特点及相关性，为语言发育落后儿童制订干预措施和疾病管理决策提供理论依据。

方法

回顾性分析172例儿童的病例资料，其中孤独症谱系障碍（autism spectrum disorder，ASD）组88例，发育性语言障碍（developmental language disorder，DLD）组34例，全面发育迟缓（global development delay，GDD）组50例。采用格里菲斯心理发展量表中文版（griffiths development scales-chinese edition，GDS-C）量表和0～6岁儿童智能发育筛查测验（the mental developmental screening test，DST）量表评估3组儿童各能区发育水平，采用K-W检验分析3组间各能区发育水平的差异，采用Spearman进行各能区之间及DST与GDS-C之间相关性分析。

结果

DLD组在GDS-C评估中各能区发育商及总商均显著高于ASD组和GDD组（

＜0.05），而ASD组总商、粗大运动、手眼协调和视觉表现发育商显著高于GDD组（

＜0.05）。ASD组和GDD组手眼协调发育商与视觉表现相关性最大，DLD组个人社会发育商与手眼协调存在的相关性最大。DST运动能区和社会适应能区发育商分别与GDS-C粗大运动和个人社会领域呈强正相关，DST智力能区发育商与GDS-C个人社会、听力语言和手眼协调均呈强正相关。

结论

ASD、DLD和GDD3种语言发育落后的儿童常见发育性疾病的智能发育有各自结构特点，智能发育筛查评估和诊断评估之间存在较高的相关性。

Abstract

Objective

To analyze the characteristics and correlation of mental development of common developmental diseases in children with delayed language development

providing a theoretical basis for the formulation of intervention measures and disease management decisions for children with language retardation.

Methods

The clinical data of 172 children were retrospectively analyzed

including 88 in autism spectrum disorder (ASD) group

34 in developmental language disorder (DLD) group and 50 in global development delay (GDD) group. Griffiths development scales-chinese edition (GDS-C) scale and the mental developmental screening test (DST) scale were used to evaluate the development level of each functional area in the three groups. K-W test was used to analyze the differences in the development levels of each energy zone among the three groups. Spearman was used to analyze the correlation between each energy region and between DST and GDS-C.

Results

In the GDS-C assessment

the development quotient and total quotient of all energy zones in the DLD group were significantly higher than those in the ASD and GDD groups (

0.05)

while the development quotient of total

gross motor

hand-eye coordination and visual performance in the ASD group were significantly higher than those in the GDD group (

0.05). The hand-eye coordination development quotient was the most correlated with visual performance in ASD and GDD groups

and the personal social development quotient was the most correlated with hand-eye coordination in DLD group. The development quotient of DST motor and social adaptive zones was strongly positively correlated with GDS-C gross motor and individual social domain

respectively. The development quotient of DST intellectual capacity was strongly correlated with GDS-C individual social

listening and language

and hand-eye coordination.

Conclusion

The mental development of children with ASD

DLD and GDD has its own structural characteristics

and there is a good correlation between the screening assessment of mental development and the diagnostic assessment.
