摘要

目的

系统评估急性低频感音神经性聋（acute low-frequency hearing loss，ALHL）患者的前庭功能特点，为临床诊断和治疗提供依据。

方法

选择40例ALHL患者作为研究组，同期招募40名健康对照组。所有受试者均接受纯音听力测试及冷热刺激试验、视频头脉冲试验（video head impulse test，vHIT）、颈性前庭诱发肌电位（cervical vestibular evoked myogenic potential，cVEMP）和眼性前庭诱发肌源性电位（ocular vestibular evoked myogenic potential，oVEMP）等前庭功能检查，对比分析ALHL患者的前庭功能损伤情况。根据治疗效果，将ALHL患者分为有效组和无效组，对两组的相关研究指标进行差异性分析，并将有统计学意义的变量纳入二元Logistic回归模型，探讨影响ALHL治疗有效性的相关因素。

结果

ALHL组在冷热试验（

0.001）、vHIT（

=0.012）、cVEMP（

0.001）和oVEMP（

0.001）检查中的异常率均显著高于对照组。vHIT异常与治疗有效率有关（

=0.025），相对于vHIT正常者，vHIT异常者治疗有效可能性减少0.840倍（OR=0.160，95%CI：0.028～0.927）。年龄、患耳侧别、耳鸣、耳闷及冷热试验、cVEMP和oVEMP异常与治疗有效率无关。

结论

ALHL患者普遍存在多发性前庭器官损害，前庭功能评估，特别是半规管高频检测（vHIT）异常是ALHL治疗无效的独立危险因素。

Abstract

Objective

This study aims to systematically evaluate the vestibular function characteristics of patients with acute low-frequency sensorineural hearing loss (ALHL) to provide a basis for clinical diagnosis and treatment.

Methods

Forty ALHL patients were included in the study group

with an equal number of healthy controls recruited at the same time. All the subjects underwent pure tone audiometry and vestibular function tests

including cold and warm stimulation tests

video head impulse test (vHIT)

cervical vestibular evoked myogenic potential(cVEMP) and ocular vestibular evoked myogenic potential (oVEMP). Vestibular function impairment in the ALHL patients was compared and analyzed. Based on treatment outcomes

the ALHL patients were categorized into effective and ineffective groups

and a differential analysis of relevant research indicators was conducted for the both groups. Subsequently

statistically significant variables were incorporated into a binary logistic regression model to identify factors influencing the efficacy of ALHL treatment.

Results

The abnormal rates in the ALHL group were significantly higher than those in the control group for the cold and warm tests (

cVEMP (

and oVEMP (

0.001). vHIT abnormalities were associated with treatment efficacy (

=0.025)

indicating that patients with abnormal vHIT had an 0.840 times lower likelihood of treatment effectiveness compared to those with normal results (OR=0.160

Conclusion

ALHL patients commonly exhibit multiple vestibular organ impairments. Vestibular function assessments particularly abnormalities detected in high-frequency vHIT of the semicircular canals

are an independent risk factor for ineffective treatment of ALHL.
