      Clinical applications of listening effort in China

      摘要:ObjectiveExploring the impact of speech presentation level on listening effort in a group of normal hearing listeners.Methods38 adults with normal hearing were recruited, and the speech comprehension test paradigm was used with manipulations of speech rate and presentation level. The pupil dilation as the index of listening effort, speech comprehension accuracy and reaction time were measured to observe the effect of 20 dB SPL increase in presentation level on task performance outcomes.ResultsThe speech rate has a significant impact on average pupil dilation (P<0.001, ηp2=0.618). However, the sound intensity has no significant effect on the average pupil dilation (P=0.213, ηp2=0.040). Increasing the volume of speech can significantly shorten the task response time (P=0.043, ηp2=0.099), and under normal speech speed, it can significantly improve the accuracy of speech comprehension (P<0.001, ηp2=0.374).ConclusionIncreasing the intensity of speech presentation by 20 dB SPL within the range of auditory comfort threshold in normal hearing individuals can increase listening efficiency, however, this effect may not be due to listening effort change by altering audibility or arousal, but rather a result of altering auditory processing speed.

      摘要:The relationship between tinnitus and listening effort has received increasing attention. So far, the majority of related research is empirical, using various measurement methods of listening effort combined with evaluation methods of tinnitus to explore the impact of tinnitus on listening effort in the clinical tinnitus population. There are not many theoretical insights focusing on the relationship between the two topics. This review aims to extract and summarize the potential links between tinnitus and listening effort from a cognitive function perspective, and to propose a verifiable theoretical framework for the tinnitus-listening effort relationship mediated by cognition.

      摘要:ObjectiveTo explore whether hearing aid use influences the listening effort trajectory as a function of task difficulty levels, and the relationship between the subjective listening effort ratings induced by auditory tasks and by daily life experience recall.MethodsTwenty-four adult binaural hearing aids wearers were asked to rate their listening effort under a speeded speech understanding task and a speech in noise understanding task on a 0-10 analogue scale with and without their hearing aids. Each task consisted of 6 difficulty levels. They were also asked to rate the aided listening effort based on their experiences in 6 typical daily life scenarios. A Linear regression model was applied to analyze the listening effort trajectory. Pearson correlation was performed between the two subjective evaluations of listening effort.ResultsThe intercept of the regression line of listening effort without hearing aids was significantly higher than that with hearing aids (i.e., hearing aid use significantly reduced listening effort in easier listening conditions, P<0.05). The slope of the regression line of listening effort with hearing aids was significantly higher than that without hearing aids (i.e., the aided listening effort trajectory was more similar to that of normal hearing individuals, P<0.05). The decrease in listening effort was associated with the number of hours of daily hearing aid use (i.e., the longer the daily wearing time, the more the listening effort decreased, P<0.05). The subjective listening effort induced by auditory tasks was not related to that induced by daily listening experiences (P>0.05).ConclusionIn a typical audiology clinic setting, auditory task with speech rate and signal-to-noise ratio manipulations can be applied to effectively induce subjective listening effort ratings and compare listening effort allocation patterns between with and without hearing aids use. The daily wearing time of hearing aids is an important influencing factor in obtaining listening effort reduction benefits from hearing aids use. When evaluating subjective listening effort, it is necessary to consider the differences in induction scenarios.

      摘要:ObjectiveExploring the relationship of the tinnitus perception and experience with subjective listening effort.MethodsSeventy-two newly diagnosed adult patients with tinnitus as their primary complaint were recruited. Extended pure tone test, tinnitus matching test, residual inhibition test, Tinnitus Function Index (TFI) and two listening effort questions were administered. Participants were divided into groups based on the averaged threshold across the extended high frequencies and the residual inhibition outcomes. Pearson correlations and ANOVA analyses were performed on the data.ResultsNo significant difference was found in tinnitus perception (i.e., loudness level and frequency) between the normal hearing group and the hearing-impaired group (P>0.05), nor in the scores of listening effort question 1 and 2 between the groups (P>0.05). There was a statistically significant difference (P<0.05) in the hearing subscores of TFI between the two groups. The listening effort question scores were significantly correlated with different TFI subscales between the normal hearing and hearing-impaired groups. There was no significant difference in the total TFI score among subjects with different residual inhibition effects (P>0.05), but the residual inhibition outcome had a significantly impact on the score of the listening effort question 1 (P<0.05).ConclusionThe perception of the tinnitus loudness and frequency may not affect patients' ability or motivation to allocate less listening effort on the tinnitus. In contrast, the experience of the impact of tinnitus on daily function and emotions may play an important role in effective listening effort management.

      摘要:ObjectiveTo explore the specific impact of wearing a mask on the engagement of listening effort in speech perception among preschool hearing-impaired children.MethodsUse 3 (mask types: no mask, surgical mask, transparent mask) × 2 (audiovisual level: pure listening, audiovisual) two factors repeated measurement experimental design. This study measured the working memory score of 29 preschool hearing-impaired children using a serial recall task to understand their listening effort fitness engagement.ResultsUnder pure auditory conditions, the average score of preschool hearing-impaired children without masks was 33.91%, which was not significantly different from the surgical mask test score of 30.32% (P=0.315), but significantly higher than the average score of 25.72% for transparent masks (P<0.001), and the surgical mask score was significantly higher than the transparent mask score (P=0.045). At the audio-visual level, the average score of preschool hearing-impaired children without wearing masks was 41.15%, significantly higher than the 28.02% score of surgical masks (P<0.001) and the 32.90% score of transparent masks (P<0.001), and the score of transparent masks was significantly higher than that of surgical masks (P=0.030).ConclusionWearing a mask can lead to an increase in the level of listening effort invested in speech perception by preschool hearing-impaired children, but under the conditions of surgical masks for pure listening and transparent masks for audiovisual use, children are relatively less engaged in listening effort.

      摘要:Listening effort is a dimension of auditory assessment that should not be overlooked; it makes up for the lack of traditional speech tests, and guides exploring the auditory system from behavioural, cognitive and neuroscience perspectives. Difficulty in listening to noise is a common problem in people with hearing loss. School-age children spend more than 70% of their time in school listening in the presence of background noise, which can lead to auditory allotment overdrafts that can affect classroom performance, academics, social activities, and even mental health. This paper aims to explore advances in research on listening effort in school-age children, including currently available methods for assessing children's listening effort and new findings currently available in experiments. It will also explore the future of clinical applications of listening effort in children, to provide ideas for more researchers to study listening effort in children.

      摘要:It is urgent to pay attention to the education and rehabilitation of hearing-impaired students in special education schools. This study reviews the empirical studies on intervention of hearing-impaired students' reading ability abroad in recent years from three aspects: the status quo of intervention research on the reading ability of hearing-impaired students, the progress of intervention content research and the influencing factors of intervention effect, systematically introduce intervention methods on the reading ability of hearing-impaired children in special education schools, and summarize the influencing factors of intervention research. This paper provides some reference for the improvement of reading ability of hearing-impaired students in special education schools.

      摘要:Due to being lively and active characteristics, school-age children have always been a high incidence of vocal nodules. This paper analyzs the effects of voice training (voice therapy) combined with drugs and behavioral intervention, the effects of voice therapy and the causative factors of vocal nodules in school-age children from CNKI, ProQuest, ScienceDirect and other databases.After combing and analyzing the research results of the treatment of vocal nodules in children in the past 20 years, it is found that: Voice therapy is gradually used in the treatment of vocal nodules in school-age children, voice evaluation and acoustic analysis are used in the treatment of vocal nodules in children. in recent years, there have been many studies on the effect of voice training (voice therapy) combined with Chinese and Western drugs in the treatment of vocal nodules in children, but the research on voice therapy and its effect are obviously insufficient and there is a very lack of professional voice therapist. studies abroad related to the pathogenic factors of children's vocal nodules and special studies on voice therapy, such as behavioral therapy, resonance therapy, symptomatic therapy and therapeutic effect research, are relatively diverse in contents and have certain systematic characteristics.

      摘要:Otosclerosis is an autosomal dominant disease with reduced penetrance, but approximately 40%-50% of clinical cases are sporadic without a family history of the disease. Many studies suggest that otosclerosis is a complex genetic disorder caused by the combined effects of genetic and environmental factors. Genetic factors include molecular pathways, such as bone remodeling, immune pathways, inflammatory factors, and endocrine pathways, which play a role in the development of otosclerosis.

      摘要:ObjectiveTo evaluate the cognitive function and speech recognition of presbycusis, analyze the influence of cognitive function on speech recognition, and provide the possibility for increasing the intervention and rehabilitation program of presbycusis.Methods106 subjects with presbycusis were selected, and their cognitive functions were measured by Montreal cognitive assessment scale (MoCA), and their auditory and speech function was tested. According to the score of the scale, all subjects were divided into normal cognitive group and cognitive impairment group, with 53 cases in each group. The differences of speech recognition functions between the two groups were observed, and the influence of cognitive function on speech recognition was analyzed.Results(1) There were significant differences in visual space, executive ability, attention, language, abstraction, delayed recall and orientation between normal cognitive group and cognitive impairment group (P<0.05). (2) The maximum speech recognition rate in the normal cognitive group was significantly higher than that in the cognitive impairment group (P<0.05). (3) The score of MoCA scale was positively correlated with the maximum speech recognition rate (r=0.269, P<0.05). (4) There was significant difference in speech recognition rate between different groups (P<0.05).ConclusionCognitive function will affect speech recognition of presbycusis of presbycusis. With the decrease of cognitive function, speech recognition becomes weaker.

      摘要:ObjectiveTo explore the current situation and influencing factors of inclusive education literacy of kindergarten teachers in Beijing.Methods152 kindergarten teachers in Beijing were selected as the research objects to investigate their inclusive education literacy through the questionnaire of Kindergarten Teachers' Inclusive Education Literacy.Results(1) The overall score of inclusive education literacy of kindergarten teachers in Beijing was 3.51±0.71. (2) The scores of the four dimensions from high to low were the attitude towards inclusive education (3.91±0.73), skills of inclusive education (3.57±0.72), knowledge of inclusive education (3.41±0.86), ability to obtain support (3.25±0.83), and the score differences among the dimensions reached a significant level (P<0.05). (3) The overall scores and scores of all dimensions of inclusive education literacy of kindergarten teachers with bachelor's degree or above were significantly higher than those of kindergarten teachers with college degree or below (P<0.05). (4) The overall literacy scores of kindergarten teachers who received on-the-job training in special education were significantly higher than those who did not receive training (P<0.001).ConclusionFirst, the inclusive education quality of kindergarten teachers in Beijing is at a medium level. Second, the development level of kindergarten teachers with different dimensions of inclusive educational literacy in Beijing is not balanced. Third, academic background and on-the-job training of special education are two factors affecting the inclusive education quality of kindergarten teachers in Beijing.

      摘要:ObjectiveTo investigate the relationship on self-consciousness and behavioral problems of the hearing-impaired children.Methods106 hearing-impaired children aged 8 to 16 were investigated by the Piers-Harris Self-Consciousness and the Achenbach Children Behavior Checklist (CBCL).ResultsThe self-consciousness of hearing-impaired children was significantly different from Chinese childrens' on intelligence and school, physical appearance and social integration (P<0.05). On the behavioral problems of the hearing-impaired children, boy's scores were significantly higher than girl's on introversion, extroversion and total scores (P<0.05). The scores of introversion, extroversion and total in CBCL were negatively correlated with the scores of behavior, anxiety, happiness and satisfaction in self-consciousness (P<0.05).ConclusionRising the self-consciousness of hearing-impaired children can reduce their behavioral problems.

      摘要:ObjectiveTo analyze the characteristics and correlation of mental development of common developmental diseases in children with delayed language development, providing a theoretical basis for the formulation of intervention measures and disease management decisions for children with language retardation.MethodsThe clinical data of 172 children were retrospectively analyzed, including 88 in autism spectrum disorder (ASD) group, 34 in developmental language disorder (DLD) group and 50 in global development delay (GDD) group. Griffiths development scales-chinese edition (GDS-C) scale and the mental developmental screening test (DST) scale were used to evaluate the development level of each functional area in the three groups. K-W test was used to analyze the differences in the development levels of each energy zone among the three groups. Spearman was used to analyze the correlation between each energy region and between DST and GDS-C.ResultsIn the GDS-C assessment, the development quotient and total quotient of all energy zones in the DLD group were significantly higher than those in the ASD and GDD groups (P<0.05), while the development quotient of total, gross motor, hand-eye coordination and visual performance in the ASD group were significantly higher than those in the GDD group (P<0.05). The hand-eye coordination development quotient was the most correlated with visual performance in ASD and GDD groups, and the personal social development quotient was the most correlated with hand-eye coordination in DLD group. The development quotient of DST motor and social adaptive zones was strongly positively correlated with GDS-C gross motor and individual social domain, respectively. The development quotient of DST intellectual capacity was strongly correlated with GDS-C individual social, listening and language, and hand-eye coordination.ConclusionThe mental development of children with ASD, DLD and GDD has its own structural characteristics, and there is a good correlation between the screening assessment of mental development and the diagnostic assessment.

      摘要:ObjectiveTo reduce or eliminate the aggressive behavior of hearing-impaired children through integrated parent-child rehabilitation training.MethodsA hearing-impaired child with aggressive behavior in the parent-child rehabilitation class of Hangzhou Wenhui School was selected as the experimental object, and the frequency of aggressive behavior and the change of auditory ability evaluation score were compared before and after the experiment.ResultsThe number of aggressive behaviors in half day activities decreased significantly, and the score of auditory evaluation increased significantly. There was a significant difference between the frequency of aggressive behaviors and the score of auditory evaluation before and after the intervention (P<0.01).ConclusionThe integrated parent-child rehabilitation can effectively eliminate the aggressive behavior of the experimental subject and promote the development of auditory ability.

      摘要:ObjectiveTo analyze the language detail sentence test scores of 3-year-old hearing-impaired children and normal hearing children of the same age, and to provide reference for improving the effect of rehabilitation teaching.MethodsThirty cases of hearing-impaired children and thirty cases of normal hearing children were selected from rehabilitation institutions and normal kindergartens respectively. We used the language detail sentence test form of the Evaluation of Auditory Response to Speech (EARS) to test the sentence and vocabulary level, and compared the scores of the two groups.ResultsThe children's hearing condition had a significant influence on the language detail sentence test score (Tsentence (1, 58) =3.36, P <0.01; Twords (1, 58)=2.89, P<0.01), the scores of sentence and vocabulary in the normal hearing children group (Msentences=8.47, Mwords=58.23) were significantly higher than those in the hearing-impaired children group (Msentences=7.03, Mwords=56.63) (P<0.05).ConclusionCompared with normal hearing children, the syntactic and grammatical abilities of hearing-impaired children aged up to 3 years are relatively lagged behind, which may be related to the lack of language information input, and input environment for hearing-impaired children. The results of the study suggest that the corresponding training should be strengthened and applied to the life scenes.

      摘要:ObjectiveTo explore the relationship between voice emotion perception and production in hearing-impaired children and to provide guidance for the treatment.Methods76 children aged from 3 to 5 years old were asked to judge and imitate two emotions (happiness and sadness) sound of a woman from cues conveyed by expressing neutrally semantic sentences.ResultsProduction scores were lower than perception scores (P<0.001). The average production scores were extremely positively related to perception scores (P<0.001).ConclusionThere was a robust correlation between the perception and production tasks; this implies that difficulties with voice emotion perception underlie their difficulties with production.

      摘要:ObjectiveTo explore the application effect of reward language training to the rehabilitation of school-age children with functional communication disorder.Methods126 school-age children with functional communication disorder admitted to our hospital from March 2021 to March 2023 were selected and divided into control group (n=63) and study group (n=63) according to random number table method. The control group received conventional language training, and the study group received reward language training on this basis. The effects of the two groups were compared before treatment and after treatment, including one-time blowing time, blowing time, longest vocal time, oral motor function, speech articulation, speech behavior ability and articulation disorder.ResultsThe total effective rate of the study group was 93.65%, which was significantly higher than 80.95% of the control group (P<0.05). After treatment, the time of one-time blowing, blowing and longest vocalization in the two groups was longer than that before treatment, and the study group was significantly longer than the control group (P<0.05). The motor function scores of mandibular, lip and tongue in two groups were higher after treatment than before treatment, and the study group was significantly higher than the control group (P<0.05). After treatment, the speech articulation and language behavior ability scores of the two groups were higher than before treatment, and the voice handicap index (VHI) score was lower than before treatment, and the study group was significantly better than the control group (P<0.05).ConclusionReward language training is effective in the rehabilitation of school-age children with functional communication disorder and can improve their oral motor function.

      摘要:With the rehabilitation of hearing-impaired children being improved, preschool inclusive education has received enormous attention. Research has shown that preschool inclusive education plays an important role in the development of hearing-impaired children. This paper has reviewed sorts out more than 160 relevant literature, analyzed the status quo of teachers, parents, children with hearing impairment and national policies in terms of the key factors affecting preschool inclusive education, and proposes solving strategies in hope of contributing to the further exploration and reflection on this issue.

      摘要:Normal hearing children have acquired 9 types of bǎ-construction by the age of 6, which is close to the use of adult Bǎ-construction. Through the interactive experience and cognitive processing with the objective world, normal hearing children establish an Event-Domain Cognitive Model (ECM) in their minds. This model takes actions and events as the core elements. when children perceive real life with the Event-Domain composed of actions and events as the unit, and then combined with the spatial displacement cognitive schema at the physical space level, human space level, time level, social space level, etc., different types of bǎ-construction are formed. The understanding of the acquisition mechanism of bǎ-construction in normal hearing children is helpful for early diagnosis and intervention of children with speech disorder.

      摘要:Inclusive education is an equal opportunity to provide inclusive, diverse and equitable education for children with special educational needs. Hearing-impaired children have improved their hearing levels and verbal communication skills through the use of cochlear implants or hearing aids, creating the possibility of returning to universal school learning. However, hearing-impaired children will also encounter various problems and difficulties in school life and study. It is necessary for teachers, schools, families to make joint efforts to dissolve obstacles and promote the process of hearing-impaired children to receive education well and complete their studies.

      摘要:ObjectiveTo explore the effects of repetitive transcranial magnetic stimulation (rTMS) combined with comprehensive rehabilitation intervention on clinical symptoms and speech function rehabilitation of children with autism spectrum disorder (ASD).MethodsFrom August 2021 to May 2023, 141 children with ASD were randomly selected in the Rehabilitation Center of Hunan Children's Hospital and divided into the observation group (72 cases) and the control group (69 cases) by the random number table method. The control group was given conventional comprehensive rehabilitation treatment, and the observation group was given rTMS treatment on the basis of the control group. The relevant parameters and scale scores were compared. All children were followed up for at least 5 months.ResultsAfter treatment, Autism Behavior Checklist (ABC) and Childhood Autism Rating Scale (CARS) scores in the observation group were significantly lower than those in the control group (P<0.05). The scores of various factors in the neuropsychological development checklist of children aged 0 to 6 years old showed that adaptive ability, social ability and speech function scores in the observation group were significantly higher than those in the control group after treatment (P<0.05), while there was no difference in gross motor and fine motor scores between the two groups (P>0.05). At the last follow-up, the Verbal Behavior Milestones Assessment and Placement Program (VB-MAPP) scores in the observation group were significantly higher than those in the control group (P<0.05).ConclusionsrTMS combined with comprehensive rehabilitation intervention can significantly improve the symptoms of children with ASD, improve their neurological, verbal, social and perceptual abilities.
