人工耳蜗植入儿童社会互动能力研究

摘要(Abstract):

目的研究人工耳蜗植入儿童术后听觉语言康复后社会互动能力及与健听儿童的差异。方法选择我院2016年1月~2018年11月收治的40名(女19名,男21名)4~12岁人工耳蜗植入患儿为研究组,另选择我院同期听力检测的40名(女18名,男22名)4~12岁健听儿童为对照组。采用Vineland自适应行为量表对两组儿童的社会交往状况进行评估。采用CAP、SIR、Vineland自适应行为量表了解主要看护人不同文化水平和家庭收入水平对研究组儿童干预疗效的影响。结果治疗后,两组儿童同龄段Vineland适应行为量表得分均显著高于治疗前(P<0.05),而治疗后两组同年龄段Vineland适应行为量表得分无明显差异(P>0.05)。研究组主要看护人初中以下和高中文化程度者相比较,CAP得分无显著差异(P>0.05),但SIR和Vineland得分差异显著(P<0.05);主要看护人大学以上文化程度者均可显著提高患儿CAP、SIR、Vineland量表得分(P<0.05)。家庭经济情况有差异,则CAP、SIR和Vineland量表得分差异具有统计学意义(P<0.05)。结论同年龄组人工耳蜗植入儿童的社会交往状况与健听儿童相当。此外,父母受教育水平、家庭收入情况是术后言语识别与语言能力、社会互动能力的影响因素。
Objective The aim of this study was to investigate the social interaction ability of children with cochlear implants after auditory and speech rehabilitation and the differences with normal hearing children. Methods 40 cochlear implanted children aged 4 to 12 years(19 girls and 21 boys) from our hospital from January 2016 to November 2018 were enrolled as the study group, and 40 healthy children aged 4 to 12 years from our hospital were selected(18 girls and 22 boys) as the control group. The Vineland Adaptive Behavior Scale was used to evaluate the social interaction status of the two groups. CAP, SIR and Vineland Adaptive Behavior Scale were used to understand the effect of different education levels and family income levels of caregivers on the intervention effect of children in the study group. Results After treatment, the scores of the Vineland Adaptive Behavior Scale at the same age were significantly higher than those before treatment(P<0.05), but there was no significant difference in the Vineland Adaptive Behavior Scale at the same age(P>0.05). In the study group, there was no significant difference in CAP score(P>0.05), but there was significant difference in SIR and Vineland scale scores(P<0.05). The scores of CAP, SIR and Vineland scales of the children were significantly increased by caregivers with a degree of education above college level(P<0.05). If there were differences in family economic conditions, the differences in the scores of CAP, SIR and Vineland scale were statistically significant(P<0.05). Conclusion In the same age group, the social communication status of the children with cochlear implants was similar to that of the normal hearing children. In addition, parents' education levels and family income are the influencing factors of postoperative speech recognition, language ability and social interaction ability.
